The following forms are required for all sports
participation:
- A fully completed Athletic
participation / physical form is required for all athletes.
- Physical examinations are
required before students will be allowed to participate in any
school-sanctioned athletic events. Return forms completed by the
student's physician to the coach / faculty advisor responsible for the
athletic event.
- Under state law, school districts
are required to ensure that all members of school athletic teams have
accidental injury insurance taht covers medical and hospital expenses.
This insurance requirement can be met by the school district offering
insurance or other health benefits that cover medical and hospital expenses.
- Some pupils may qualify to enroll in no-cost or low-cost
local, state, or federally sponsored health insurance programs.
Information about these programs may be obtained by calling 800-367-5830
- Student athletes must turn in proof of insurance with
their athletics participation forms. This can be a photocopy of
their insurance card, or insurance policy.
- Complete the Athletics
Emergency Treatment Card which authorizes treatment in the
event of an injury or medical emergency.
In addition, student athletes and parents must read,
understand, and comply with the requirements on the following forms:
- In order to participate in athletics, students and parents need to agree to
abide by the rules and policies covered in the Athletics Eligibility form. To
indicate agreement, student athletes and parents must sign the Patrick Henry
Athletics Signature card (obtained form the coach). This card only needs to
be signed once each year.
- Review, understand, and comply with the requirements
on the CIF Athletic Ethics Form
- Review and understand the Substance Abuse code for athletes.
